超声吸引刀(CUSA)结合双极电凝在肝叶切除术中的应用研究 被引量:9

摘要 目的探讨超声吸引刀(Cut-U ltrasound Asp iration,CUSA)结合双极电凝在肝叶切除术中的技巧和可行性。方法对我院2004年7月至2005年7月间进行肝叶切除的82例肝肿瘤病人进行回顾性分析,并根据是否应用这种技术分成A、B两组,A组46例运用CUSA结合双极电凝肝叶切除,B组36例行常规肝叶切除,并比较两组疗效。结果A组46例病人均顺利恢复,而且切肝的术中出血量、术后引流量、转氨酶恢复时间、术后拔管时间、术后平均住院日和并发症发生率均比常规肝叶切除术组疗效好,二者之间有显著性差异(P<0.01)。结论运用CUSA结合双极电凝是肝叶切除术的一种实用而且有效的技术,能提高切除率,并减少并发症的发生率。 Objective To investigate the possibilitv and clinical value of the Cut-Ultrasound Aspiration combining bipolar coagulation in hepatectomy. Methods Eighty two cases with liver tumor were divided into two groups by with or without CUSA combining bipolar coagulation before operation. Forty six patients were operated by the ultrasonic scalpel combining bipolar coagulation in group A. Thirty six patients were performed hepatectomy with routine methods(group B ). The operation time, anaount of bleeding, hospitalized time of postoperation , and complication rate of the two groups were compared. Results The ultrasonic scalpel combining bipolar coagulation group was superior to the other groups in the amount of bleeding, complication rate. Conclusion It is safe anti reliable that the hepatectomy was pertormed by the ultrasonic scalpel combining bipolar coagulation
