摘要
目的:分析经口鼻面罩无创机械通气治疗急性肺栓塞合并低氧血症患者的临床价值。方法:回顾分析16例确诊的肺栓塞患者 在给予抗凝、解痉、消炎等对症处理的同时,经口鼻面罩无创机械通气纠正低氧血症的临床资料。结果:无创通气治疗后1 d,患者的 PaO_2、 呼吸频率和心率与治疗前有显著性差异(P<0. 01) ;治疗3 d 后 PaO_2、 呼吸频率和心率与治疗前有显著性差异(P<0. 01) ,且上 述数值较治疗后1 d 有进一步的改善,特别是治疗3 d 后的 PaO_2较治疗1 d 后数值有进一步的改善(P<0. 01) 。结论:无创通气应用 于急性肺栓塞伴低氧血症者可以明显纠正低氧血症,纠正酸碱平衡紊乱,减少心肺意外并发症的发生。
Objective: To study the cliniical value of non-invasive mechanical ventilation in treat-ment of patients with acute pulmonary thromboembolism and hypoxemia. Methods: 16 pulmonary thromboembolism patients,while accepting general treatment,accepting non-invasive mechanical ventilation to treat hypoxemia, were analyzed retrospectively. Results: One day later ventilating,the PaO2 ,breath rate and heart rate changed obviously(P〈0. 01 ) ;three days later ventilating, the PaO2 ,breath rate and heart rate changed obviously(P〈0. 01 ), and PaO2, breath rate and heart rate havd more change than One day later ventilating. Conclusion.. Non-invasive mechanical ventilation may treat hypoxemia obviously, redress disturbance of acid-base balance, decreasing the risk of occurrence heart-pulmonary accident.
