眼眶炎性假瘤的CT诊断

Value of CT scan in diagnosis of orbital inflammatory pseudotumor

目的探讨CT扫描对眼眶炎性假瘤的诊断价值及临床意义。方法回顾性分析经临床、病理证实的26例眼眶炎性假瘤患者的CT表现。结果CT征象有眼球突出21例,眼环增厚7例,泪腺肿大5例,眼外肌增粗16例,眶内脂肪密度增高12例,视神经增粗3例,合并鼻窦炎5例。本组可分为①弥漫型10例,表现为病变范围弥漫无明确肿块;②肿块型2例,表现为可见到边界清楚的肿块;③泪腺炎型3例,表现为仅见泪腺肿大;④肌炎型11例,表现为1条或数条眼外肌增粗、肥大。结论CT扫描对眼眶炎性假瘤的诊断有重要价值并能进一步指导临床治疗。

Objective To study the value of CT scan in diagnosis of orbital inflammatory pseudotumor. Methods Twenty-six cases diagnosed by either clinical manifestation or pathology were investigated. Results The CT signs of orbital inflammatory pseduotumor in these cases included exophthahnos （n=21）, sclera thickening（n =7）, lacrimal gland enlargement （n=5）, extraocular mucle enlargement（n=16）, orbital fat density increasing（n=12）,optic nerve thickening（n=3）,sinusitis（n=5）,etc. The CT signs can be divided in to four types： ① the widespreading type （n=10）： the performance fills the air to have no explicit lump for the pathological changes scope ;② the lump type（n=2）： the lump is clear for the boundary;③the lacrymal gland type（n=3）： the lacrymal gland is swollen greatly,which is the only sign： ④the extraocular muscle type（n=11）： it performs one or several extra ocular muscle enlarge. Conclusion CT scan are of great value in diagnosis of orbital inflammatory pseudotumor and treatment further.
