荷包缝合钳代替直线型缝合器在低位直肠癌中的应用

Purse string suture clamp and stapler for low rectal carcinoma

目的探讨安全、实用、经济的低位直肠癌保肛手术方法.方法回顾性分析48例低位直肠癌应用荷包缝合钳代替直线型缝合器关闭直肠残端,配合管状吻合器行低位保肛术的临床资料.结果低位前切除28例,超低位前切除20例.术后出现吻合口漏1例（2.1%）,无吻合口狭窄,切缘无肿瘤残留,无手术死亡.结论荷包缝合钳代替直线型缝合器安全、实用且经济,值得在基层医院推广使用.

AIM To assess the sphincter saving resection of low rectal carcinoma with purse string suture clamp and stapler. METHODS Forty-eight patients with low rectal carcinoma receiving sphincter saving resection with purse string suture clamp and tubular stapler were analyzed, retrospectively. RESULTS Low anterior resection was performed on 28 cases, and ultra-low anterior resection on 20 cases. Stomal leak was recorded only in one case（2.1% ）. There were no anastomotic stenosis and pathologic examinations revealed negative for residual tumor. CONCLUSION Sphincter saving resection with purse string suture clamp and tubular stapler is a recommendable approach to low rectal carcinoma.
