反流性食管炎X线影像与胃镜检查结果对照研究

Research on comparison between X-ray diagnosis and gastroscopy in reflux esophagitis

目的研究数字X线影像与胃镜检查在反流性食管炎诊断中的价值。方法采用数字胃肠造影与胃镜分别对406例反流性食管炎进行检查,并对两种检查结果进行对比分析。结果406例反流性食管炎X线表现特征是:轻度103例为功能性改变,钡剂滞留,食管排空缓慢,出现异常收缩波;中度245例为粘膜增粗,呈条状、颗粒状,部分病例有小溃疡形成;重度58例为食管粘膜明显增粗,紊乱,有多个小溃疡,管腔狭窄,钡剂通过受阻,B arret食管形成。胃镜检查食管粘膜充血406例,靡烂融合成片310例,溃疡96例,食管腔狭窄58例。结论数字胃肠检查对食管功能性变化,形态与病变范围,胃对食管的影响较清楚。胃镜对食管炎的腔内病变显示较为理想,特别是对早期食管炎病变的观察,同时亦可做病理检查定性。

Objective Study the diagnosis value to reflux esophagitis by X-ray examination and gastroscopy. Methods 406 cases with reflux esophagitis were examined by digital gastrointestinal radiography and gastroscopy respectively, and the results of two examination were analysed contrastively. Results Among 406 cases of esophagitis, the typical characteristic of cases by digital gastrointestinal radiography were： 103 cases with light illness history showed the functional change of esophagus, the dose of Barium kept in the esophagus, the empty of the esophagus was slow and the special wave of contract occurred; 245 cases with middle ill- ness history showed mucous membrane widened, pellet, some cases formed light ulcer, 58 cases with heavy illness history showed the wideness of mucous membrane increased obviously disordered in esophagus, there were a lot small ulcers , the esophagus turned to be narrow and the dose of Barium went through with difficulties, and Barret formed. Among 406 cases of congestive mucous membrane in esophagus by gastroscopy, 310 cases were erosion combined into pieces, 96 cases of ulcer, 58 cases as narrowness of esophagus. Conclusion The digital gastrointestinal series is good to diagnose the functional change, shape and scope of esophagus. It is clear to observe the effect of stomach to esophagus. Gastroscopy is an ideal method to observe the pathological changes, especially, it can observe the early pathological changes of esophagitis, it also helps to diagnose on the pathology.
