摘要
目的将数据库技术应用于心血管外科临床资料的管理以及我科承担的全军医药卫生基金课题的实施与开展。方法应用Microsoft Access XP软件、VB6.0、Microsoft SQL建立心内直视手术后ALI/ARDS专业数据库,通过建立专业字典与临床资料表格实现了数据库的专业性;通过控件属性的设置建立了实用的数据录入窗体,减少文字输入,减轻工作量;通过SQL命令语句实现数据库的查询功能;通过与统计软件的联结实现高效能的统计分析功能。结果采用该实用的专业数据库,顺利完成了体外循环心内直视手术后血气分析的变化特点与临床意义、体外循环后ALI/ARDS病例临床分析、体外循环后长时间呼吸机支持与ARDS发生的危险因素分析等多项研究,取得了良好的效果。结论我们自行构建的心脏外科专业数据库,可较全面地收集心内直视手术病人临床资料,从而提高了临床研究的效率,也增强了临床研究的科学性。专业数据库作用的发挥有赖于临床医疗工作中的规范化与制度化。
Objectives To utilize the professional database to collect the clinic data in cardiovascular department and conduct the assigned project funded by military medical fund. Methods Microsoft Access XP software. VB6.0, Microsoft SQI. were used to create the cardiovascular database. First, the professional dictionaries and tables were established. Secondly, practical window was created to input the patients' clinic data, and the effect setting was established to alert the mistakes in data inputting. Thirdly, SQL command sentences were compiled to implement the single of muhiple conditional query, and the statistics analysis was performed via the inner interface of SPSS software. Results The professional database was practical and feasible to collect the clinic data and achieved admirable results. By using this database, we reviewed the characteristics and clinic significance of blood gas analysis, summarized the clinic characteristics and treatment of ARDS following cardiopulmonary bypass (CPB), investigated the risk factors of prolonged mechanical ventilation following CPB, and investigated the risk factors of ARDS following CPB. Conclusion The pro- fessional database created by our workteam is practical and feasible to collect the clinic data following open heart surgery and improve the efficiency of the project and ensure the accuracy of the studies. The formalization and standardization of medical treatment help the database exert its powerful functions.
