脾虚湿盛型特应性皮炎患者部分免疫学特点研究

Some Immunologic Characters in patients with Atopic dermatitis in Damp Abundance Due to Spleen Asthenia Syndrome

[目的]探讨脾虚湿盛型特应性皮炎(AtopicDermatitis,AD)患者部分免疫学特点。[方法]采用ELISA方法检测门诊61例中医辨证属于特应性皮炎(AD)患者和20例健康人的血清总免疫球蛋白E(IgE),采用流式细胞仪检测外周血T细胞亚群及T辅助细胞(Th)Th1T/h2状态,利用RT-PCR方法检测外周血单个核细胞干扰素γ(IFN-γ)及白介素-4(IL-4)mRNA表达。[结果A]D患者血清总IgE水平显著高于健康对照组(P<0.01),且外周血CD3+T、CD4+T百分比显著高于健康对照组(P<0.01或P<0.05),而CD8+T百分比低于对照组(P<0.05),患者外周血Th1细胞百分比低于健康对照组(P<0.01),而Th2百分比高于健康对照组(P<0.01),IL-4mRNA表达显著高于对照组(P<0.01)。[结论]血清总IgE的升高,T淋巴细胞功能的紊乱,Th1T/h2的失衡构成了脾虚湿盛型AD患者的部分免疫学特点。

[Objcclive] To investigate the immune character in patients with atopic dermatitis in damp abundance due to splenic asthenia syndrome（AD）. [Methods] Sixty-one patients with atopic dermatitis were enrolled from out-patient clinic. Twenty healthy persons were as control. The level of serum total IgE was determined by ELISA The T cell subgroup of peripheral blood and T accessory cell （Th1/Th2） were determined by flow cytometry. The IFN-γ, and IL-4 mRNA expression in peripheral blood mononuclear cells were determined by RT-PCR. [Result] The level of serum total IgE in patients with atopic dermatitis was higher than that in control group （P〈0.01）. The percentage of CD3^＋CF and CD4^＋CF wase significantly higher than that in control group（P〈0.01 and P〈0.05）. But the percentage of CD8^＋T was remarkablely lower than that in control group（P〈0.05）. The percentage of Thl cell in the peripheral blood was significantly lower than that in control group（P〈0.01）. The percentage of Th2 cell in the peripheral blood was significantly higher than that in control group（P〈0.01）. The expression of IL-4 mRNA was significantly higher than that in control group（P〈0.01）. [Condusion] The high level of serum total IgE, the disorder of T cell function and the imbalance of Th1/Th2 were the immune character in patients with atopic dermatitis in damp abundance due to spleen asthenia syfidrome.
