摘要
目的应用高频超声检测系统性红斑狼疮(SLE)患者的颈总动脉血流动力学及结构的有关参数,无创性评价SLE患者早期动脉粥样硬化的发展进程及血管弹性。方法研究对象包括42例系统性红斑狼疮患者(依是否合并高血压分为SLE1组、SLE2组)和20例健康对照者。应用高频超声检测并计算颈总动脉内中膜复合体厚度、弹性指数(Ep)、僵硬指数(β)及最大剪切率(SR)。结果SLE1组、SLE2组内中膜复合体增厚,Ep指数、β指数增高,颈总动脉SR降低,且与对照组比较均有显著性差异(P<0.05)。结论SLE患者动脉粥样硬化病变早期在颈总动脉血流动力学及结构方面即有反映,表现为内中膜复合体增厚,最大剪切率减低,弹性减低。高频超声在SLE患者血管病变的诊断中具有重要的实用价值。
Objective To observe parameters of hemodynamic and structure of the common carotid arterial in patients with systemic lupus erythematosus (SLE) and evaluate the early progression of atherosclerosis and arterial elasticity with high resolution ultrasonography. Methods Forty-two patients with SLE (SLE1 group with hypertension, SLE2 group without hypertension) and twenty healthy subjects were studied. The intima media thickness (IMT) of common carotid artery, pressure strain elastic modulus (Ep), pressure independent stiffness index (13) and shear rate (SR) were measured. Results Compared with the control group, IMT of common carotid artery was thicker, the Ep and 13 increased, while the SR was lower (P〈0.05) in patients with SLE. Conclusion In patients with SLE, IMT of common carotid artery was increased, the Ep and 13 higher, while the SR was lower. High resolution ultrasonography is important in dignosis of vascular complications in patients with SLE.
