Effect of Allopurinol on the Function of Blood Vessel Endothelium in Patients with Essential Hypertension Complicating Hyperuricemia

摘要 目的:研究别嘌呤醇对原发性高血压合并高尿酸血症患者血管内皮功能的影响。方法:65例原发性高血压合并高尿酸血 症患者随机分为治疗组与对照组,治疗组在对照组减少钠盐摄入、适当锻炼、减轻体重等非药物治疗的基础上给予别嘌呤醇 100mg,2次/d,治疗4wk。比较2组治疗前、后血尿酸水平、肱动脉血流依赖性舒张功能(FMD)、基础状态下肱动脉内径、一氧化氮 (NO)、血管性假性血友病因子(vWF)的变化。结果:与治疗前比较,治疗组治疗后血清尿酸水平降低,vWF水平降低,NO浓度升 高,FMD明显升高,均有统计学意义(P<0.05);基础状态下肱动脉内径有减小趋势,但无统计学意义。对照组治疗前、后比较,各 项检测指标均无统计学意义(P>0.05)。结论:别嘌呤醇可改善原发性高血压合并高尿酸血症患者的血管内皮功能。 OBJECTIVE: To study the effect of allopurinol on the function of blood vessel endothelium in patients with essential hypertension complicating hyperuricemia. METHODS: 65 cases with essential hypertension complicating hyperuricemia were randomized into treatment and control group. The control group was given the classic nonpharmacotherapy: reduced intake of sodium, more exercise, weight loss etc. The treatment group was treated with 100mg allopurinol b .i .d for 4 weeks besides the classical non - pharmacotherapy as stated for the control group. Serum uric acid level, brachial artery flow - mediated diastolic (FMD) function, the base internal diameter of brachial artery, nitrogen oxide (NO), yon Willebrand' s factor(vWF) were compared between two groups before and after treatment. RESULTS: As compared with prior treatment, the treatment group had decreased level of uric acid and decreased vWF level, increased level of NO and a significantly increased level of FMD, all had statistical significances(P〈 0.05)and the base internal diameter of brachial artery diminished yet of no statistical significance. All the detection indexes for the control group were of no statistical significance after treatment as compared with prior treatment (P 〉 0.05) . CONCLUSION: Allopurinol can ameliorate the function of blood vessel endothelium in patients with essential hypertension complicating hyperuricemia.
