圆锥角膜穿透性角膜移植术后远期疗效动态分析

Curative effect analysis long after penetrating keratoplasty of keratoconus

目的动态分析圆锥角膜穿透性角膜移植术后的远期效果。方法收集本院22例27只眼因圆锥角膜行部分穿透性角膜移植术12m至192m的患者,分别进行视力、屈光度、散光及角膜地形图的检查,并对数据进行分析。结果术前与术后1周,拆线前、后与术后2y～5y视力比较,差异有显著意义(P<0.01或P<0.05)。术后2y～5y与术后5y～16y相比差异无显著性(P>0.05)。术后各随访阶段散光度比较差异无显著意义(P>0.05)。圆锥角膜术后两种类型角膜地形图参数差异有显著意义(P<0.01)。结论圆锥角膜穿透性角膜移植术后疗效显著,无复发倾向,穿透性角膜移植术是圆锥角膜重要的治疗方法。

Objective To analysis the curative effect long after penetrating keratoplasty of kerato-conus. Methods Twenty - seven eyes of 22 patients 12 to 192 months after partial penetrating keratoplasty were examined by vision, diopter, astigmism and corneal topography, and the data was analysed. Results There were significant divergence of vision among pre - operative, 1 week after operation, before taking out stitches, after taking out stitches （P〈0.01 or P〈0.05）. The vision of 2-5 years after PKP did not differ from that of 5-16 years after the operation （P 〉 0.05 ）. There was no difference of astigmism at different time （ P 〉0.05）. Two types of keratoconus after PKP have remarkable divergence of post - operative corneal topography （P〈0.01）. Conclusions Keratoconus after PKP has significant results and no trend of recur-rent, PKP is the effective methods treating keratoconus.
