Herbst矫治器治疗安氏Ⅱ类1分类错的颞下颌关节磁共振研究

摘要 目的:定量研究Herbst矫治器治疗后颞下颌关节的改建。方法:选择Ⅱ类1分类患者18例,治疗前后拍摄4张左右颞下颌关节磁共振片,在磁共振片上测量髁状突表面软骨增生的量,并用Kurita方法分析髁状突、关节盘及关节窝之间的位置关系在治疗前后的改变。结果:①髁状突后上方表面软骨有明显的增生迹象:增生量最大为1.3mm,最小为0.6mm,平均0.93mm;②治疗前后髁状突、关节盘及关节窝之间的位置关系有所改变:TC/TP、TD/TP改变有统计学意义(P<0.05),TD/TC改变无统计学意义(P>0.05)。结论:Herbst矫治器可刺激髁状突、关节盘及关节窝产生适应性改建。 Objective: To study the temporomandibular joint remodeling quantitatively after herbst treatment. Methods: Eighteen patients exhibiting class Ⅱ division 1 malocclusion were selected. For each subject, 4 magnetic resonance imagings of both temporomandibular joints were taken before and after treatment. On MRI, the growth of condylar cartilage was measured and the relationship between condyle,articular disk,and glenoid fossa were analyzed by Kurita, s analysis method.Results: (1)Signs of condylar cartilage remodeling were seen at the posterosuperior border:the maximum thickness of the bright sign was 1.3 mm,the minimum was 0.6 mm and the mean was 0.93 mm; (2) the relationship between condyle,articular disk,and glenoid fossa changed: concerning TC/TP and TD/TP, there was statistic difference between before and after treatment, concerning TD/TC,there was no statistic difference between before and after treatment. Conclusion: Herbst treatment can stimulate condyle,articular disk,and glenoid fossa to remodel adaptively.
