胰岛素泵在2型糖尿病围手术期的应用

Clinical applications of Insulin pump in perioperative period of patients with type 2 diabetes mellitus

目的比较连续皮下胰岛素输注治疗(胰岛素泵CSII)和多次皮下注射胰岛素治疗(MSII)用于围手术期2型糖尿病的疗效和安全性。方法对68例需要胰岛素治疗的2型糖尿病择期手术患者的资料进行总结,治疗前血糖在11.8～23.5mmol/L,分别采用胰岛素泵CSII和MSII,CSII组30例,MSII组38例。结果CSII组治疗达到理想血糖控制的疗效和所需的时间明显短于MSII组,胰岛素用量减少,低血糖发生率低,手术后的感染率下降。结论CSII对控制围手术期2型糖尿病是一种安全、有效、方便和准确的方法,可缩短术前准备时间。

Objective To compare the efficacy and safety between continuous subcutaneous insulin infusion（insulin pump CSⅡ） and multiple subcutaneous insulin injection （MSⅡ） in treating perioperative type 2 diabetic patients. Methods Insulin therapy were performed in 68 type 2 diabetic patients with elective operation, their blood sugar was 11.8～23.5mmol/L. 30 patients were given continuous subcutaneous insulin infusion threatment （CSⅡ） and 38 patients were given repeated multiple subcutaneous insulin injection （MSⅡ）. Results The time needed to gain a good effect by adopting continuous subcutaneous insulin infusion is less than repeated multiple subcutaneous insulin injection .The dosage of insulin and hypoglycemia incidence in CSII were less than those in MSⅡ. Postoperative infection rate was decreased. Conclusions CSⅡ is a safe, effective and convenient method to control perioperative type 2 diabetes mellitus. This shorten the pre-operative time.
