解剖钢板内固定治疗儿童股骨转子间骨折

Treatment of femoral intertrochanteric fractures in children with anatomical bone plates

[目的]探讨解剖钢板治疗儿童股骨转子间骨折的临床效果。[方法]采用解剖钢板对15例儿童股骨转子间骨折进行手术固定治疗。平均随访时间20个月(8～36个月)。[结果]本组骨折均在14周内愈合。1例出现髋内翻。无1例发生股骨头缺血性坏死、骨不连、骺板早闭和肢体短缩。参照刘尚礼疗效评定标准,14例疗效优良,优良率93.3%。[结论]解剖钢板治疗儿童股骨转子间骨折符合生物力学观点,具有创伤小、手术操作简便、固定牢固、患髋能够早期功能锻炼等优点,是目前较好的固定方法。

[Objective] To discuss the clinical effect of anatomical bone plates in treating femoral intertrochanteric fractures in children.[ Method] Anatomical bone plates were applied to 15 children patients with femoral intertrochanteric fractures. The following up period was 8～36 months with an average of 20 months. [ Result] All fractures knitted within 14 weeks postoperatively. One case had hip adduction. No avascular necrosis of femoral head, none-union of bone, epiphyseal plat＇s early closure or limb shortening was founds. According to LIU＇s criteria, 14 cases achieved excellent and good results（93.3%）.[Conclusion] Anatomical bone plate treating intertrochanteric fractures in children meets the biomechanical demands. This method hasobvious advantages of less trauma, easy operation, more reliable fixation, as well as early physical exercises. It is by far a good fixation method in the treatment of femoral intertrochanteric fractures in children.
