酒精性肝病大鼠创伤弧菌脓毒症各脏器的超微结构观察

Changes of the ultrastructure of the organs during the process of vibrio vulnificus infection in the rats with alcohol liver disease

目的观察酒精性肝病大鼠创伤弧菌脓毒症重要脏器的超微结构变化,探讨研究创伤弧菌的致病性。方法采用酒精灌胃联合自由饮酒法制做大鼠酒精性肝病模型,取成模大鼠12只,分成2组,实验组6×108cfu/mL浓度创伤弧菌右下肢皮下注射感染肝病大鼠;对照组等量生理盐水右下肢皮下注射。48 h后颈动脉取血液及患肢病变组织进行细菌培养,留取肝脏、心脏、肺脏、肾脏、脾脏、右下肢肌肉、大脑等脏器组织0.1 cm×0.1 cm×0.1 cm置电镜固定液,超薄切片观察超微结构。结果实验组大鼠12 h后,皮温升高,右下肢明显肿胀、皮肤发紫,精神萎靡,活动减少,呼吸急促,均在48 h内濒临死亡,血液及患肢病变组织培养出创伤弧菌,电镜下见心肌、肺、肝、肾等组织的部分实质细胞内和间质中有创伤弧菌生长,肝脏、心脏、肺脏、肾脏、脾脏、右下肢肌肉、大脑存在不同程度超微结构改变,部分血管内皮细胞损伤及基膜断裂溶解,线粒体、内质网等细胞器损伤明显,其中以下肢骨骼肌、肺、肝脏的超微结构改变尤为明显,脾脏的改变相对较轻。结论创伤弧菌感染酒精性肝病大鼠后导致多个脏器的实质细胞及间质超微结构损伤,致使多器官功能障碍甚或衰竭,是导致机体快速死亡的重要原因。

Objective Changes of major organs infection in the rats with alcohol liver disease. Methods ultrastructure were observed to investigate the pathogenicity of Vibrio vulnificus Alcohol liver disease was induced by freely drinking and perfusing stomach with alcohol. 12 rats with alcohol liver disease were randomly divided into two equal groups. The first group were injected intraperitoneally with Vibrio vulnificus dosage of 6×10^8 cfu/mL（0.125 mL/100 g） in the right legs, the second group were injected intraperitoneally with Saline. The blood and tissues of the infected legs were obtained and cultured after 48 hours. The ultrastructures were studied. Result In the forst group temperature of the skin heightened, the right leg swelled, the skin cyanosis, spiritlessness, the activity reduced and breath was quick at 12 hours after being affected by Vibrio vulnificus. Vibrio vulnificus were cultured from the blood and tissues of the infected legs. There were obvious abnormal changes in the ultrastructure of the main organs. Conclusions Vibrio vulnificus infection in the rats with alcohol liver disease brings obvious changes in the ultrastructure of the main organs and comes out MODS. This is the main reason for quick death.
