全髋关节置换关节脱位的危险因素评估及护理干预

The risk factors for the hip joint dislocation after THA and the nursing intervention

目的探讨全髋关节置换并发关节脱位的危险因素评估方法及护理干预的效果。方法利用历史资料1997年3月-2002年6月全髋关节置换的病人226例未采用潜在脱位危险因素评估为未评估组;2002年7月-2004年12月312例全髋关节置换的病人实施术前全面潜在脱位危险因素评估,加强健康教育、重视肌力训练、规范功能康复锻炼的方法和生活护理知识指导为评估组。结果全髋关节置换术后关节脱位发生率,有手术史病人全髋关节置换术后关节脱位发生率分别经!2检验,均有显著性差异,P均<0.05。关节脱位与护理因素有关两组比较经!2检验,虽无显著性差异,P>0.05,但发生率评估组比未评估组低。结论重视病人潜在并发关节脱位危险因素评估,是有效预防并发症的前提,提高护理质量的关键。

Objective To investigate the risk factors of the hip joint dislocation after THA and the effect of nursing intervention. Methods Two hundred and twenty-six eases of total hip arthroplasty （THA） from March 1997 to June 2002, who had not been evaluated concerning risk factors of hip joint dislocation, was set to Group A and another 312 cases from July 2002 to Deeember 2004, who had been comprehensively evaluated, to Group B, where health education was enhanced, training of myodynamia focused on, functional rehabilitation exercise and nursing instruction on daily life standardized. Results The rate of hip joint dislocation in Group A was significantly different from that in Group A, and so it was with relationship between dislocation and operation experience, statistically by X^2 test （P〈0.05）. Conclusion The evaluation of risk factors for latent hip dislocation is both the prerequisite for effective prevention against complications and the key to raise the quality of nursing.
