免疫抑制剂致肾移植患者十二指肠溃疡穿孔(附1例报告并文献复习)

Management on perforation of the upper gastrointestinal tract following renal transplantation

目的:探讨肾移植术后免疫抑制剂致十二指肠溃疡穿孔的处理方法。方法:结合有关文献复习对1例此种患者的临床资料进行分析。结果:该例十二指肠溃疡穿孔行修补术后,出现穿孔愈合延迟、严重呼吸道感染,经治疗穿孔及呼吸道感染治愈而移植肾失功能。结论:免疫抑制剂应用是引起肾移植患者发生上消化道溃疡穿孔的主要因素;此种患者预后欠佳,早期诊断,应综合各器官情况,进行有效处理。

Objective：To investigate how to deal with the perforation of the upper gastrointestinal tract follow ing renal transplantation. Methods：Combining the literature, the clinical data of 1 case of patient with upper gas trointestinal tract perforation following renal transplantation was analyzed and the management for this complica tion was summarized. Results： Although this patient had recovered from delayed healing of perforation accompanied with severe respiratory infection, the graft had lost its function. Conclusions： The perforation in the upper gastrointestinal tract following renal transplantation is mainly caused by the administration of immunosuppressive agents. The prognosis of perforation in upper gastrointestinal tract following kidney transplantation is poor. Therefore. early diagnosis and effective treatment are key points for this complication.
