摘要
目的应用多普勒组织速度成像技术(TVI)测定冠心病左室壁二尖瓣环处运动速度,为临床特别是手术麻醉前提供冠心病左心功能的定量分析指标。方法分别测定冠心病组及对照组二尖瓣环-室间隔交界处、二尖瓣环-侧壁交界处、二尖瓣环-前壁交界处、二尖瓣环-下壁交界处的左室心肌多普勒运动速度参数,并进行两组间参数的差异性比较。结果组织多普勒频谱分别在舒张早期及舒张晚期各有两个反向波峰,即E峰和A峰,收缩期有一个正向速度波峰为S峰。冠心病组与正常组相比收缩期速度峰值VS明显降低(P<0·01),舒张早期峰值速度(VE)也明显降低(P<0·01),舒张晚期峰值速度(VA)则相对较高。VE/VA比值明显降低。结论TVI技术能准确定量冠心病患者舒张期及收缩期左室壁运动状态,从而为判定左心功能提供了一个良好的观察指标,特别对手术麻醉患者术前评估手术麻醉耐受性具有重要意义。
Objective To measure the velocity of mitral valve hoop part beside the Left ventricular wall in coronary artery disease by Doppler tissue velocity imagery(TVI) technique. And to provide a quantitative analysis index of left ventricular function for clinic.Methods The objectives were divided into two groups: 30 people in group A were normal, 30 patients in group B wereCAD patients in clinic. The velocity parameters of the left ventricular myocardium in juncture of mitral annulus and interventricular septum , juncture of mitral annulus and lateral wall, juncture of mitral annulus and anterior wall , juncture of mitral annulus and inferior wall A were measured respectively. And the difference between the groups was analyzed.Results The tissue Doppler frequency showed that there were two baekware peaks in early diastolic phase and late diastolic phase: E spike and A spike; there was a norientation peak in systolic stage: S spike. The PSV in systolic stage decreased significantly in CAD group compared with the control group( P 〈 0.01). the peak of early diastolic stage velocity(VE) decreased significantly too( P 〈 0.01 ), while the peak of later diastolic stage velocity (VA) was higher, VE/VA ratio decreased significantly too. Conclusion TVI technique could accurately quantitate the movement of left ventricular wall in diastolic phase and systolic phase in CAD, which provides a satisfactory index for assessment of the LV function, and has important significance for assessment the tolerance of patients for anaesthesia and operation before anaesthesia.
