摘要
目的探讨背向散射积分定量鉴别诊断离体子宫正常肌层和腺肌病病灶的可行性。方法收集新鲜手术离体的子宫肌层组织块标本,定位进行二维和背向散射成像并存档,然后联机分析,记录平均背向散射积分值(AII);标本超声成像后立即进行固定,常规石蜡包埋、切片和HE染色,并由资深病理医师作出病理学诊断;按病理诊断结果将标化的平均背向散射积分值(AII%)划分为正常肌层组和腺肌病病灶组。结果经病理学检查确诊标本中正常肌层组34例及腺肌病病灶组32例;两组患者年龄无统计学差异(P>0.05);正常肌层组AII%(3.71±0.57)高于子宫腺肌病病灶组AII%(3.00±0.33),两者差异有显著性意义(P<0.05)。结论利用背向散射积分技术定量鉴别诊断子宫正常肌层与腺肌病病灶是可行的,该方法在妇科临床超声诊断中有潜在价值。
Objective To study the feasibility of integrated backscatter (IBS) for quantitative differential diagnosis between the normal myometrium and the focus of adenomyosis in vitro. Methods Two-dimension and IBS scanning were performed immediately on fresh myometrium specimens after being excised. The imaging of sections were kept into archives, then used for the values of average image intensity (AII) by the analysis programs of the same imaging system. The specimens were fixed immediately after ultrasound scanning, then were embedded by paraffin, sliced and stained by haematoxylin-eosin in the routine way. After the slices were read by senior pathologist, the calibrated average image intensity(AII%) of IBS imaging was divided into two groups as the normal myometrium and the adenomyosis according to their pathological diagnosis. Results Among the specimens, 32 cases were diagnosed as the focus of adenomyosis and 34 cases as the normal myometrium by pathologic examination. There was no significant difference between the average age of patients of two groups( P 〈0.05) ; However, there was significant difference between the AII% of the normal myometrium and the focus of adenomyosis, which were 3.71 ± 0. 57 and 3.00 ± 0.33 respectively. Conclusions It is feasible to use IBS for quantitative diagnosis between the normal myometrium and the focus of adenomyosis in vitro, and it is potential to apply this technique in clinical ultrasound diagnosis of gynecology.
