Kugel自膨式补片和Perfix网塞修补腹股沟疝比较研究

A comparison of Kugel self-expanding patch herniorrhaphy vs Perfix mesh plug herniorrhaphy for inguinal hernia

目的比较Kugel补片和Perfix网塞修补腹股沟疝的效果.方法Kugel补片修补组(下称Kugel组)41例,Perfix网塞修补组(下称Perfix组)45例.对两组患者术前基本情况,手术时间,术后早期反应,住院时间,术后并发症及复发率进行了比较.结果Kugel组患者平均年龄及伴随疾病显著高于和多于Perfix组(P＜0.05),Kugel组术后伤口肿胀反应及痛感均显著低于Perfix组(P＜0.05).平均随访9个月(1～16个月),两组均无术后复发.结论与Perfix网塞修补术相比,Kugel补片修补术具有伤口疼痛和肿胀轻的优点,但术者需具有熟悉的腹股沟区解剖知识及丰富的手术经验才能完成好该手术.

Objective To compare Kugel self- expanding patch herniorrhaphy vs Perfix mesh plug herniorrhaphy for inguinal hernia. Methods Forty - one cases of inguinal hernia were subjected to Kugel patch herniorrhaphy （Kugel Group） and 45 cases to Perfix mesh plug （Perfix Group）. A comparison was made in characteristics of patients, operating time, early postoperative response, hospital stay, postoperative complications and recurrence rate between the two groups. Results Mean age and concomitant diseases in Kugel group were significantly older and more than those in Perfix group （ P 〈 0.05 ）. Postoperative swelling response and pain of the wound in Kugel group were remarkably milder than in Perfix group（ P 〈 0.05 ）. Neither Kugel group nor Perfix group were found having hernia recurrence at a mean follow up of 9 month （ranging from 1 -- 16 months）. Conclusion Comparing with Perfix mesh plug herniorrhaphy, Kugel patch herniorrhaphy has less pain and swelling response of the wound, but it is imperative for operator to have both thorough understanding of the inguinal anatomy and rich experience with herniorrhaphy in order to achieve success of this technique.
