医务人员对新农合定点医疗机构服务质量的评价

Evaluation of service quality by medical staff in assigned medical institutions carrying new cooperative medical system

目的探讨医务人员对新农合定点医疗机构服务质量的评价情况,为有关卫生主管部门提高新农合定点医疗机构服务质量提供科学依据。方法以问卷方式调查,整群随机抽取部分云南省寻甸县医务人员对新农合定点医疗机构服务质量的意见和建议。结果医务人员对各定点医疗机构医疗技术水平、可信度、总体满意度、收费、卫生资源配置情况以及选择新农合定点医疗机构就诊的评价分布差异分别都有统计学意义 (P<0．001)。经非条件Logistic回归分析表明,村卫生室就医方便、路程近、一般满意度水平是少数医务人员选择就医的主要原因,而医生医技水平差等其它因素导致大多数医务人员不选择村卫生室就诊。结论村卫生室和乡镇卫生院总体满意度较低,卫生资源配置不合理,县级医疗机构收费过高。建议有关卫生主管部门大力提高乡村医生的医技水平,合理配置卫生资源,降低和统一新农合定点医疗机构的收费。

Objective Through probing into the appraisement of medical staff on the service quality of New - type Rural Cooperative Medical Care in assigned medical institutions, so as to provide scientific evidences for the health competent department concerned to improve the service quality of these institutions, Methods We randomly sampled a part of medical staff by the cluster sampling, who were of medical staff in Xindian county, Yunnan province, and investigated their comments and proposals about the service quality of assigned medical institutions by the questionnaire method. Results There were statistical significant differences （ P 〈 0. 001 ） between the respondents＇ relative appraisal distributions of medical technical level, reliability, the total satisfactory degree of various assiged medical institutions, the allocations of health resources, medical charge and the selecting visit among the various assigned medical institutions. Through analyzed the data by Non -conditional Logistic Regression Analysis, we found that the convenience of receiving medical treatment, short distance and common satisfactory degree of village infirmaries were the principal reasons of attracting a few respondents to visit them. However, we also found that major respondents didn＇t visit the village infirmaries because there were other factors such as doctors＇ poorly medical technical level, etc. Conclusions The total satisfactory degrees of village infirmaries and town health center were very low. The allocations of health resources were unreasonable. The medical charge of county - level hospitals were too high. It was suggested that the health competent department concerned energetically improve the medical technical level of rural doctors, reasonably allocate health resources, depreciate and unify the medical charge of assigned medical institutions.
