发绀型先天性心脏病围手术期氧代谢特点临床初探 被引量:3

摘要 目的:探索发绀型先天性心脏病围手术期氧代谢变化规律,从而为围手术期处理提供依据。方法:用SwanGanz四腔漂浮导管及惠普M1205A监护仪分析围手术期氧代谢指标变化。结果:术后72h内氧供(DO2)与氧耗(VO2)始终表现为直线相关关系,P<0.05。术后4hVO2最高,同时氧摄取率(ERO2)明显增加,而DO2并未显著增加。术后8~12h心排出量指数(CI)降至最低,为围手术期72h最低谷,此时段动脉血乳酸(LAC)表现为反弹性增高。术后12h内,尽管CI仍保持在正常范围内,但氧代谢指标已发生明显变化,表现为LAC升高,动静脉氧含量差(CavO2)增加,混合静脉氧饱和度(SvO2)降低。结论:氧代谢变化早于血流动力学改变。发绀型先天性心脏病围手术期72h均表现为病理性氧供依赖状态,易发生并发症或器官功能衰竭。术后4h为偿还氧债高峰期,机体开始表现出氧耗增加与氧供相对不足的矛盾。术后8~12h为发绀型先天性心脏病围手术期最危险时段,需及时进行干预性治疗以纠正氧代谢失衡,从而降低并发症的发生率及病死率。
