腹部闭合损伤345例临床分析

目的分析腹部闭合损伤的临床特点,探讨如何提高腹部闭合损伤的诊断及抢救成功率.方法回顾性分析我院1994～2004年收治345例腹部闭合损伤的临床资料.本组手术治疗281例,保守治疗55例,在术前准备中死亡9例.结果保守治疗组53例治愈,2例死亡;手术治疗组264例治愈,17例死亡,35例发生并发症.结论腹腔内大出血、复合伤及救治延误是死亡的主要原因.对此类患者应及时准确的手术治疗,部分轻度腹部损伤患者可以保守治疗;严重复合伤应多科室协作救治.

Objective To analyze the clinical characteristics of blunt abdominal trauma and to investigate how to improve the success rate of earlier diagnosis and emergency resuseitations. Methods The clinical data of 345 eases of such patient at out hospital between 1994 to 2004 were collected and analyzed retrospectively. 281 of them were treated operatively, and the remaining 55 were subjected to conservative treatment, another 9 patients died in preparation before operation. Results In the conservative group, 53 eases were cured with 2 eases died. In the operative group264 eases were cured with 17 eases died and various complications occurred in 35 cases. Conclusion In severe abdominal hemorrhage and multiple trauma, delayed treatment would be important factors resulting in death. This patients should be accurately operated in time. Parts of the patients who had slight abdominal injury may be treated by conservatively. Multiple departments cooperation should be made in treatment of the patient with severe multiple trauma.
