抗核糖体P蛋白抗体对系统性红斑狼疮的临床意义

Significance of detection of anti-ribosomal P protein antibody in SLE

目的探讨抗核糖体P蛋白抗体(ARPA)在检测系统性红斑狼疮(SLE)中的临床意义,了解其与SLE以及与其他自身抗体的关系。方法用酶免疫斑点(条带)实验检测SLE患者、其他风湿性疾病对照组(包括干燥综合征,多发性肌炎,进行性硬化征,皮肌炎,类风湿性关节炎和强直性脊柱炎)和正常对照组血清中ARPA的阳性率;分析ARPA与它们的关系。结果SLE患者血清中的ARPA阳性率显著高于其他风湿性疾病对照组和正常对照组(P<0.05)。ARPA阳性组中神经系统损害和关节炎的发病率明显高于ARPA阴性组(P<0.05)。ARPA的表达与抗SSA、抗SSB和抗nRNP抗体表达相关(P<0.05),而与ANA、抗dsDNA和抗sm抗体的相关性不强(P>0.05)。结论ARPA是SLE血清学诊断的一个重要指标,它与SLE的神经系统损伤和关节炎的发病有关;与抗SSA、抗SSB和抗nRNP具有相关性,联合检测能对SLE的诊断提供重要依据。

[Objective] To investigate the clinical sinificance of determing anti-rlbosomal p protein antibody in SLE and to analysis the relationship between ARPA and activity of SLE and other auto-antibodies. [Methods] Enzyme-immuno-Blot method was used to detect the ARPA in serum of 126 SLE patients and 121 patient with other rheumatic diseases（including primary sjogren sydrome, polymyositis, etc.） and 35 healthy people. [Results] The positive rate of ARPA in SLE was obviously higher than patients with other diseases and health. And the incidences of neurological lesions and arthritis were more common in ARPA positive SLE patients. The expression of ARPA was correlated to anti-SSA, anti-SSB, anti-nRNP, but not to ANA anti-dsDNA and anti-sm. [Conclusions] ARPA was one of the important serum markers in the diagnosis of SLE. The detection of ARPA with other auto-antibodies （especially anti-SSA,anti-SSB and anti-nRNP ） together is of great significance. Forthermore ,it has a close relationship of neurological lesions and arthritis.
