摘要
目的探讨开放性胫骨折并软组织缺损治疗方法.方法采用骨折有效固定并早期采用局部皮瓣或游离皮瓣行创面覆盖治疗Gustilo ⅢB型开放性胫骨折20例.结果本组病例随访期限为10～32个月,平均16个月.游离肌皮瓣4例全部成活,局部皮瓣转移成活16例,术后并发感染3例.骨折愈合所需时间平均为4.5～5.5个月.结论在Gustilo ⅢB型开放性胫骨折治疗中进行有效骨折固定的同时关键应进行早期软组织覆盖,这是治疗该型胫骨折的有效方法.
Objective To explore the therapeutic method of open tibial fracture complicated with soft tissue defects. Methods 20 patients with Gustilo Ⅲ B open tibial fracture were treated by fracture fixation and early wound surface coverage with local skin flap or free skin flap. Results All patients had been followed up for 10-32 months（meaning 16 months） . The resuits showed that free mesculocutaneous flaps were performed in 4 cases, who were all survived; local skin flap grafting survived in 15 cases, postoperative infection found in 3 cases. The mean time of wound healing was 4.5-5.5 months. Conclusion Effective fracture fixation and early soft tissue coverage are the key measures for treatment of Gustilo Ⅲ B open tibial fracture, and it is effective therapeutic method to treat tibial fracture.
