基层医院严重多器官损伤的救治探讨

Discusion about emergency treament of severe trauma of multiple organs in basal hospital

目的总结分析基层医院对严重多器官损伤的救治特点。方法:对我院近三年来,已收治的148例严重多器官损伤进行回顾性的分析。结果治愈117例,治愈率79%,死亡31例,死亡率21%。死亡原因:①严重损伤合并失血性休克6例;②手术后死亡17例,其中严重颅脑损伤合并其他器官损伤11例,血气胸6例;③误诊,延误手术时机8例。结论交通事故伤、机械工业伤是主要伤因。早期正确判断伤情,及时实施确定性手术是降低死亡率,有效提高严重创伤的抢救效果。

Objective To summarize and analyze the rescue features of severe multiple organ injuries in basal hospital.Methods From January 2000 to December 2003,148 cases with severe multiple organ injuries were analyzed retrospectively.Results In this group, 117 cases with severe multiple organ injuries were cured.The cure rate was 79%.31 cases died.The mortality was 21%.Cause of death：①Severe injuries associated with hemorrhagic shock 6 cases.②postoperative death 17 cases, including severe cranio cereabral injuries associated with other organ injury 11 cases, hemopneumothorax 6 cases. ③Missed diagnosis and delayed operative timing 8 cases.Conchlsion Traffic accident and mechanical industrial injuries are the main cause of injury.Early correct diagnosis and in time surgical management are very important to reduce mortality and improve the rescue effects for severe multiple organ injuries.
