63例滑膜肉瘤的临床分析

Clinical Analysis on 63 Patients with Synoviosarcoma

目的分析不同方法治疗骨髓内瘤对预后的影响。方法分析63例滑膜肉瘤患者的发病年龄、部位、病史、X线表现、病理特点和治疗方法。结果发现手术加放疗组、单纯手术组和单纯放疗组的3、5年生存率分别为55.6%、40.7%;45.8%、29.2%和8.3%、0。局部复发率分别为11.1%、54.2%和25%(P<0.05)。结论患者年龄对预后影响不大,瘤体越大,预后越差,治疗失败重要原因之一是血行转移(44.4%)。早期作出正确的诊断,积极采取对肿瘤广泛切除,区域肿大淋巴结清除,结合术前术后放疗的综合治疗方法,是提高治疗效果的关健。

Objective The factors of influencing prognosis on 63 cases of synoviosarcoma treated by various methods was analyzed. Methods A retropective study was performed in 63 cases of synovial sarcoma with the patient＇s age,medical history, radiographic, pathological manifestations and remedial method. Results The 3-and 5-year survival rates were 55.6% and 40.70% in postoperative radiotherapy plus surgery group,and 45.8% and 29.2% in surgery alone group,and 8.3% and 0 in radiotherapy alone group. Local recurrence rates were 11.1% and 54.2% and 25% in the above three groups（P〈0.05）. Conclusion The patient＇s age did not related to prognosis. The larger of the tumour,the worse of the prognosis. 44.4% patients died of distant metastasis. The authors suggested that combined using extension synovectomy, removing regional enlarged lymph nodes, postoperative radiotherapy could reach better results.
