摘要
目的 研究细胞外基质中纤维黏连蛋白、Ⅳ型胶原蛋白、层黏连蛋白和整合素β1在人非小细胞肺癌组织中 的表达特征及意义。方法 采用免疫组化En Vision法,检测57例非小细胞肺癌组织中细胞外基质和整合素β1的 表达。结果 在非小细胞肺癌中纤维黏连蛋白、Ⅳ型胶原蛋白、层黏连蛋白广泛(≥50%切片区域)表达率分别为 80.7%、71.9%和42.1%,整合素β1阳性表达率为43.9%。纤维黏连蛋白、Ⅳ型胶原蛋白、层黏连蛋白和整合素β1 在57例肿瘤标本中阳性表达率显著高于在23例正常肺组织的阳性表达率(P<O.05)。在非小细胞肺癌中,层黏 连蛋白在中高分化组的阳性表达率显著高于低分化组(P<0.05)。Ⅳ型胶原蛋白在肺腺癌表达率显著高于在肺鳞 癌的表达率(P<0.05);层黏连蛋白在肺鳞癌的表达率显著高于在肺腺癌表达率(P<0.05)。无淋巴结转移组的层 黏连蛋白的表达率显著高于淋巴结转移组(P<0.05),整合素β1在淋巴结转移组表达率显著高于无转移组(P< O.05)。结论 细胞外基质和整合素β1在非小细胞肺癌组织中有高表达;层黏连蛋白高表达可能提示肿瘤恶性程 度较低及淋巴结转移发生低,并与鳞癌发生有关;Ⅳ型胶原蛋白高表达可能与腺癌发生有关;整合素β1高表达则可 能促进癌细胞侵袭转移。
Objective Objective To study the expressing features and significance of extracellular matrix (ECM) such as fibronectin, collagen IV, laminin and integrinβ1 in human non-small cell lung cancer(NSCLC). Methods Expression of ECM and integrinβ1 in 57 NSCLC specimens were evaluated by immuno-histochemistry (Envision's method). Results In NSCLC specimens, extensive expression (≥50 % of section area)of fibronectin , collagen Ⅳ and laminin were 80.7 %, 71.9 % and 42, 1% respectively , positive expression of integriβ1 was 43.9 %. The expression rates of fibronectin, collagen Ⅳ, laminin and integrinβ1 in 57 NSCLC specimens were significantly higher than that in 23 normal lung specimens (P〈0.05). The expression rate of laminin in well and moderately differenciated NSCLC specimens was higher than that in poorly differenciated ones(P 〈 0.05). Collagen Ⅳ was more often seen in adenocarcinoma specimens than in squamous cell carcinoma ones(P〈 0.05). Laminin was more frequently seen in squamous cell carcinoma specimens than in adenocarcinoma ones(P〈 0.05). The expression rate of laminin with lymph nodes metastasis was lower than that without lymph nodes metastasis and the expression rate of integrinβ1 with lymph nodes metastasis was higher( P 〈 0.05). Conelusion ECM and integrinβ1 were highly expressed in NSCLC. It was likely that over-expression of laminin imply low malignancy , lymph nodes metastasis and was associated with the occurrence of squamous cell carcinoma. Over-expression of collagen Ⅳ might be related to the occurrence of ad lnVaslveness Over-expression of integrinβ1 might promote tumor cell invasiveness.
