风湿性心脏病并重度心功能不全80例的手术治疗

Rheumatic Heart Disease and Heart Failure Surgical Cure

目的:探讨风湿性心脏病并重度心功能不全机械瓣替换术的手术治疗经验。方法:回顾性分析2001年至2004年在体外循环下机械瓣替换治疗风湿性心脏病并重度心功能不全80例。结果:均在充分术前准备全麻体外循环下行机械瓣替换术。术后17例出现并发症但病人无围术期死亡。所有病人临床症状缓解,心功能及全身情况均有明显改善,近期效果良好。结论:外科手术机械瓣替换是治疗风湿性心脏病并重度心功能不全的有效方法。术前、术中、术后广义的心肌保护是手术成功降低手术死亡率的重要环节。防止术后出血、血栓形成及感染性心内膜炎等严重并发症的发生是提高长期生存率的关键。

Objective： To study experience of the surgical treatment of rheumatic heart disease and heart failure. Method： From 2001 to 2004, 80 cases with rheumatic heart disease and heart failure underwent operation for cardiac valve replacement were selected and analyzed. Result： All 80 cases with rheumatic heart disease and heart failure received artificial mechanical heart valve replacement under hypothermic anaesthesia with cardiopulmonary bypass. Although complication of 17caseas happened to approach in the early postoperative period, but it is reversible. Heart function all of them improved markedly. Conclusion： Artificial mechanical heart valve replacement in the patients with rheumatic heart disease and heart failure was a good efficient method.
