羟乙基淀粉和不同血液成分输入对食管癌患者术后T淋巴细胞亚群及自然杀伤细胞的影响 被引量:1

摘要 目的探讨术中输入羟乙基淀粉、红细胞悬液及血浆对食管癌患者术后T淋巴细胞亚群及自然杀伤细胞的影响。方法48名食管癌患者,分别随机分为4组,盐水组,羟乙基淀粉组,红细胞悬液组,血浆组,每组12名,分别于术前、术后d1、d5、d10、d15测定外周血T淋巴细胞亚群(CD3^+、CD4^+、CD8^+)及自然杀伤细胞(CD16+56^+)的变化。结果4组患者术后d1:CD3^+,CD4^+,CD8^+,CD16+56^+显著低于术前(P<0.05);与术前相比,术后d5:输血浆组CD3^+,CD4^+,CD8^+,CD4^+/CD8^+,CD16+56^+显著降低(P<0.05),红细胞悬液组CD3+显著降低(P<0.05),3组及输红细胞悬液组其他各项指标均无显著变化(P>0.05);术后d10,血浆组CD16+56+仍较低(P<0.05)。组间比较,术后d1、d5输血浆组CD16+56^+显著低于盐水组和羟乙基淀粉组(P<0.05);术后d5,血浆组CD4^+显著低于盐水组和羟乙基淀粉组(P<0.05)。结论食管癌术后可出现一个可逆性细胞免疫功能抑制过程,输血浆可加重免疫抑制,而输红细胞悬液对机体的免疫抑制较轻,羟乙基淀粉输入对机体免疫功能无明显影响。
