清瘀通塞汤结合电刺激小脑顶核治疗脑梗死急性期40例临床观察

Clinical observation on Qingyu Tongsai decoction(清瘀通塞汤) combined with electrical stimulation on the top core of cerebellum in treating 40 cases with acute cerebral infarction

目的:观察清瘀通塞汤结合电刺激小脑顶核治疗脑梗死急性期的疗效。方法:将80例急性脑梗死患者随机分为两组:治疗组40例采用清瘀通塞汤〔方剂:天竺黄10 g,全栝楼30 g,枳实10 g,生大黄(后下)10 g,葛根20 g,川芎15 g,水蛭9 g,丹参20 g;水煎,每日1剂,早晚分服〕结合电刺激小脑顶核(每次通电30 m in,每日治疗2次)治疗;对照组40例以低分子右旋糖酐500 m l加丹参注射液20 m l(每日1次)治疗,两组均以15 d为1个疗程。疗程结束后观察患者症状、体征和血液流变学指标变化。结果:治疗组的基本痊愈和显著进步率明显高于对照组(62.5%比32.5%,χ2=7.22,P<0.01);血液流变学各指标治疗组明显好于对照组(P均<0.05)。结论:清瘀通塞汤结合电刺激小脑顶核治疗急性脑梗死在临床疗效、血液流变学等方面优于单用西药对照组。

Objective ： To observe the curative effect of Qingyu Tongsai decoction （清瘀通塞汤 ） combined with electrical stimulation on the top core of cerebellum on acute cerebral infarction （ACI）. Methods： Eighty patients with ACI were randomly divided into two groups. Forty cases of treatment group were treated with Qingyu Tongsai decoction （concretio silicea bambusae （天竺黄） 10 g, whole trichosanthes kirilowii （全栝楼） 30 g, fructus aurantii immaturus （枳实） 10 g, raw rhubarb （生大黄） 10 g, pueraria lobata （葛根） 20 g, ligusticum chuanxiong （川芎） 15 g, hirudo （水蛭） 9 g, salvia miltiorrhiza （丹参） 20 g,two times each day] combined with electrical stimulation on the top core of cerebellum. Forty cases of the control group were treated with 500 ml low molecule dextran combined with salvia miltiorrhiza injection 20 ml intravenous infusion. The therapeutic course was 15 days in both groups. The signs, symptoms and hemorrheological indexes were observed after one course. Results： The cured and markedly improved rate was higher （62.5% vs. 32.5%, x^2=7.22, P〈0. 01） and hemorrheological indexes were improved （all P〈0.05） in the treatment group than those in the control group. Conclusion： The clinical curative effect and hemorrheological indexes are improved in patients treated with Qingyu Tongsai decoction combined with electrical stimulation on the top core of cerebellum.
