梗阻性黄疸外周血和门静脉血甲状腺激素水平的测定

Clinical study on thyroid hormone of peripheral vein blood and portal vein blood in patients with obstructive jaundice

测定20例梗阻性黄疸(A组)和25例单纯胆囊结石患者(B组)外周血和门静脉血甲状腺激素水平。结果发现:A组外周血和门静脉血中T_3、TSH显著低于B组,T_4无显著性差异。A组门静脉血中T_M、T_G显著高于B组,外周血中无显著性差异。相关分析显示TB、BTP与T_3呈显著负相关。结果表明梗阻性黄疸时不仅影响外周血和门静脉血甲状腺激素水平,而且影响甲状腺激素的反馈调节。

This article reported thyroid hormone of peripheral vein blood and portal vein blood in 20 patients with obstructive jaundice (group A) and 25 cases of simple gallbladder stones (group B). The results showed that 3. 5, 3'-triiodothyronine (T_3) and thyroid stimulating hormone (TSH) of peripheral vein blood and portal vein blood in group A were significantly lower than that in group B (P<0.01), 3, 5, 3,5'-tetraiodothyronine (T_4)was not obviously different between group A and group B (P>0.05). Thyroid micromosal antibody (TM)and thyroglobulin (TG) of portal vein blood in group A were significantly higher than those in group B (P<0.01). TM and TG in peripheral vein blood were not obviously different between group A and group B (P>0.05). T_4 in peripheral vein blood of group A was significantly higher than that in portal vein blood of group B. TSH in peripheral vein blood was obviously lower than that in portal vein blood of group B(P<0.01). The regressive analysis indicated that serum total bilirubin (TB) and biliary tract pressure (BTP) were negatively related with T_3 (P<0.01). The results indicated that obstructive jaundice not only affected thyroid hormone of peripheral vein blood and portal vein blood but also influenced feedback regulation of thyroid hormone.
