摘要
通过检索MEDLINE和Cochrane图书馆,了解钙通道阻滞剂的作用机制,对其应用于脑卒中一、二级预防的证据进行总结和评价。结果发现,钙通道阻滞剂除具有降低血压作用以外,还有抗动脉粥样硬化的作用,能够降低中风的发生率,在脑卒中一级预防中有一定作用,但也有增加心脏病变包括心肌梗塞和心衰的风险。目前尚无证据证明钙通道阻滞剂在脑卒中二级预防中的效果,因此不推荐钙通道阻滞剂作为脑卒中预防的一线药物。
We searched MEDLINE and The Cochrane Library to find high quality evidence about Ca^2＋ channel blocker in primary or secondary stroke prevention and summarized the avaliable evidence. The results show that in addition to the effect on hypertension, Ca^2＋ channel blocker has antiartherosclerotic effect and can reduce the frequency rate of stroke. It has played an important role in primary stroke prevention. But concomitantly it can increase the risk of heart disease and as yet there is no evidence on secondary stroke prevention. Accordingly, Ca^2＋ channel blockers should not be recommended as the first-fine medicine for stroke prevention.
