急性心肌梗死院前静脉溶栓疗效观察

Observation curative effect of pre-hospital intravenous thrombolysis in acute myocardial infarction

目的探讨急性心肌梗死院前静脉溶栓的疗效与护理要点.方法实验组(20例)采用院前1 h内静脉溶栓治疗,对照组(20例)1～6 h溶栓,两组均用尿激酶静滴.结果实验组中有18例再通,再通率为90%,死亡1例;对照组中有12例再通,再通率为60%,死亡3例.两组冠脉再通率与死亡率差异均有显著意义(分别P＜0.01、P＜0.001).结论急性心肌梗死院前静脉溶栓疗效显著,安全可行,可降低并发症与病死率.

Objective To study the curative effect and the nursing of the intravenous thrombolysis to prehospital in acute myocardial infarction. Methods The experiment group（20 cases） adopted the intravenous thrombolysis the pre-hospital in 1 hour. The control group（20 cases） adopted the intravenous thrombolysis in 1- 6 hours. Two groups were given urokinase 1500000U ＋ sodium chloride injection 100ml drip in vein. Results The experiment group had 18 cases to rethrough,rate of rethrough was 90 percent, 1 case died. The control group had 12 cases to rethrough, rate of rethrough was 60 percent,3 cases died. To compare the two groups of patients, the rates of the coronary rethrough and mortality showed significant difference（ P 〈 0.01, P 〈 0.001 ）. Conclusion Pre-hospital intravenous thrombolysis in acute myocardial infarction has significant curative effect and is safety. It can lower the compliations and rate of death illness.
