Epley复位法治疗良性发作位置性眩晕疗效观察

Effectiveness of Treatment of Benign Paroxysmal Positional Vertigo by Epley Maneuver

目的对比观察Ep ley复位法与西比灵治疗良性发作性位置性眩晕的临床疗效。方法观察28例良性发作性位置性眩晕患者,其中13例予以Ep ley复位法治疗,15例以西比灵治疗,对比两组1天、3天症状缓解率,随访治疗后1周内眩晕症状再次发生率、1个月后眩晕完全缓解率。结果Ep ley治疗组与对照组治疗1天后症状缓解率为10/13 vs 6/15(P<0.05),3天后症状缓解率为12/13 vs 9/15(P<0.05)。治疗后1周内眩晕症状再次发生率4/13 vs 8/15(P>0.05)。治疗后1个月,眩晕完全缓解率为11/13 vs 7/15(P<0.05),DHT阴性11/13 vs 6/15(P<0.05)。结论Ep ley法简单易学,操作方便,疗效优于常规抗眩晕药物治疗,值得推广。

Objective To compare the effectiveness of Epley maneuver and sibelium in treating benign paroxysmal positional vertigo（BPPV）. Methods 28 consecutive cases were studied perspectively and randomly, in which 13 cases were treated by Epley maneuver and the others 15 cases were treated by taking sibelium 5mg per night. The cure rates at 1 day ,3 days,a week ,a month of two group were compared. A follow - up was conducted for a month. Results The cure rates of Epley maneuver group were higher than that of sibelium group at 1 day（ 10/13 vs 8/15, P 〈0.05） and 3 days（12/13 vs 9/15, P 〈0.05） ;There was no difference in recurrence rate of two group in a week （4/13 vs 8/15, P 〉 0.05 ） , but there was a higher cure rate in Epley maneuver group in a month （ 11/ 13 vs 7/15, P 〈0.05） ,DHT negative（11/13 vs 6/15, P 〈0.05）. Conclusion Epley maneuver had a significantly higher efficacy rate in patients with BPPV than sibelium, It is safe and worthy to eb popularized in the treatment of BPPV.
