天幕上下间隙的显微外科解剖和手术入路 被引量:2

摘要 松果体区和三脑室后部位置深,血管复杂密集,向来被认为是对神经外科医生极具挑战性的区域。此区病变的手术入路经典的有Poppen和Krause入路,近年来经天幕上下间隙又有旁正中幕下小脑上入路、经小脑上经天幕入路、联合天幕上下经横窦入路、经枕经双侧天幕入路等几种手术入路,利用天幕上下间隙从不同方向、范围显露松果体区病变。充分利用天幕上下自然间隙是减少手术并发症,提高切除率的关键。熟悉间隙内桥静脉、硬脑膜窦和四叠体池内神经血管结构是更好进行此区手术的基础。
