体表潮气呼吸模式评价中重度慢性阻塞性肺疾病患者的气流限制

摘要 目的探讨用呼吸电感体积描记法(R IP)监测潮气呼吸时的体表呼吸模式能否评价阻塞性肺疾病(COPD)的气流限制。方法重度COPD患者13例、中度COPD患者10例和11名健康者(对照组)均接受肺通气功能和R IP检查。结果与对照组比较中度患者体表呼吸模式无差异;而重度患者的Ti、Ti/Ttot、TPTEF/TE指标差异有显著性。TPTEF/TE和FEV1.0/FVC、FEV1.0%均具有独立的线性相关。结论体表监测潮气呼吸模式如TPTEF/TE可评价重度COPD患者的气流限制。 Objective To investigate the breathing pattern of ribcage and abdomen on rest breathing in patients with moderate and severe chronic obstructive pulmonary disease(COPD). Methods 23 COPD patients subdivided into 13 in severe group(SG) and 10 in moderate group(MG) ,and 11 health persons as control group(CG) ,received pulmonary function tests and respiratory inductive plethysmography(RIP). Results Compared to CG,the breathing pattern in MG has no significant differences, however, Ti,Ti/Ttot,TPTEF/TE were significant different in SG,TPTEF/TE has an independent regressional relationship with FEV1.0/FVC,FEV1.0%. Conclusion The breathing pattern of ribcage and abdomen on rest breathing, especially TPTEF/TE, can evaluate the airflow limitation of severe COPD.
