应用Seldinger技术放置心包引流管的临床观察

Clinical observation of Seldinger technique in pericardial catheter drainage

目的评价心包置管引流治疗心包积液的安全性与实用性。方法16例心包积液住院患者,应用Seldinger技术,经皮穿刺心包腔内置中心静脉导管进行间断抽液或药物治疗。结果16例患者在床边不需二维超声引导均一次穿刺成功,顺利放置心包引流管,无一例出现心肌或其他脏器损伤,2例穿刺口皮肤轻微渗液和红肿,经处理后消失,留置导管时间为5～45d。结论应用Seldinger技术心包穿刺置入中心静脉导管引流治疗心包积液的方法简单、安全、实用、成功率高。

Objective To study the safety and practicality of pericardial catheter drainage in pericardial effusion. Methods A total of 16 patients with pericardial effusion were withdrawn intermittenly by Seldinger technique with percutaneous pericardial indwelling and the central venous catheter or drug administration. Results Sixteen patients were successfully inserted pericardial drainage in one time at bedside without uhrasoundgraphic guidance and none of myocardial or other organ injury were found. The slight infiltration of fluid and red swollen occurred in 2 cases but disappeared after management. The catheter was indwelled for 5 - 45 days. Conclusion The application of Seldinger technique in pericardial puncture and indwelling of central venous tube drainage in pericardial effusion is practical, safe and effective with high successful rate.
