摘要 目的探讨颈内动脉(ICA)超声检查预诊冠心病的价值。方法用超声观察临床确诊的冠心病患者的ICA,并与部分冠脉造影(CAG)对比。根据临床表现和CAG结果分组;扫查双侧颈内动脉(ICA);观察动脉内径(D)、内-中膜厚度(IMT)、有无斑块及数量、是否钙化、彩色血流是否缺损、血流速度(收缩期Vs、舒张期Vd、平均血流速度Vm)、搏动指数(PI)、阻力指数(RI)。结果本研究显示,心肌梗死组表现为动脉内径增大、血流速度减慢,特别是Vd和Vm的减慢。斑块形成早期,血管造影可有假阴性,高频超声在小斑块的检出中较血管造影更为敏感。结论ICA的IMT增加或内-中膜粗糙,回声不均或增强,是动脉粥样硬化的早期表现,超声检查ICA对预诊冠心病有一定的应用价值。 Objective To evaluate coronary atherosclerosis early by two-side internal carotid artery (ICA) ultrasonic examination and to find out a more ideal indication for the early prediction of artherosclerosis (AS). Methods To observe the two-side ICA of the patients with clinical diagnosing coronary heart disease,and compare with the partial patients with the coronary artery angiography (CAG). By observing with echocardiography the internal diameter of the ICA (ID), internal and middle membrane thickness (IMT), we can discover if plaques to be observed or calcified, and if the velocity of blood flow (systolic velocity Vs, diastolic velocity Vd, mean velocity Vm), pulsation index (PI) and resistant index (RI) of the two-side ICA to be normal or not. Results The internal diameter of ICA augments; the velocity of blood flow becomes slower. Small plaques are found more sensitive by high-frequency ultrasound uhan by CAG. Conelusions The increase or becoming rough and the resonance becoming strong of the IMT in the ICA are the early manifestations of the artherosclerosis. ICA is valuable in curing the coronary heart disease.
