摘要
目的:探讨带锁髓内钉并自体髂骨移植治疗股骨干粉碎性骨折的临床疗效。方法:对2001年3月 ̄2005年3月的70例粉碎性股骨干骨折患者进行回顾性分析,其中骨移植组43例,非植骨组27例。采用切开复位、带锁髓内钉扩髓固定。结果:骨移植组患者平均住院时间(48.5±9.3)天,显著少于非植骨组的(56.4±10.4)天(P<0.05)。骨折愈合率为95.3%,显著高于非植骨组的74.1%(P<0.05)。骨折平均愈合时间为(6.0±1.2)月,显著少于非植骨组的(7.6±1.9)月(P<0.01)。两组术后感染率无显著区别。结论:带锁髓内钉并自体髂骨移植治疗能显著减少股骨干粉碎性骨折的愈合时间,提高骨折愈合率。
Objective: To evaluate the clinical effects of interlocking intramedullary nailing and bone grafting in the treatment of comminuted fractures of femoral shaft.Methods:Seventy patients suffering from comminuted fractures of femoral shaft were included into this experiment.These patients were treated with interlocking intramedullary nail from March 2001 to March 2005. According to self-bone grafting treatment, all these patients were classified into two groups:bone grafting group and non graftubg group.Forty-three patients in the bone grafting group were treated with self-bone grafting.Results:The mean length of stay and the average healing time of bone grafting group were (48.5±9.3) days and (6.0±1.2) months respectively, which were significantly less than those of the non grafting group(P〈0.05). The healing rate of the bone grafting group was 95.3%, which was significant higher than that of the non grafting group (P〈0.01).The infection rate of two groups did not have any difference. Conclusion: Interlocking intramedullary nail and bone grafting can not only effectively accelerate fracture healing but also raise the healing rate of the comminuted fractures of femoral shaft.
