摘要
根据颞下颌关节紊乱综合征（ｔｅｍｐｏｒｏｍａｎｄｉｂｕｌａｒｊｏｉｎｔｄｉｓｔｕｒｂａｎｃｅｓｙｎｄｒｏｍｅ，ＴＭＪＤＳ）患者的关节液内存在免疫和炎症物质等研究成果，改进传统的关节封闭术，用关节腔冲洗－封闭疗法治疗４３例ＴＭＪＤＳ，取得了良好的治疗效果，并以传统的封闭法和单纯关节腔冲洗法作配对比较，证明其差异有显著性（Ｐ值分别小于０．０１和０．０５）。为探讨治疗机理，还对８例患者作了治疗前后关节液肿瘤坏死因子（ＴＮＦ）测定，其差异有高度显著性（Ｐ＜０．００５）。认为关节腔冲洗和糖皮质激素封闭有协同或累加作用。关节腔冲洗－封闭疗法操作方便，疗效好。
Accordingtotheresearchthatthereweresomeimmuneandinflammatorysubstancesinthesynovialfluidoftemporomandibularjointdisturbancesyndrome(TMJDS),weimprovedthetraditionalintraarticularinjectionanddevelopedanintraarticularirrigation-injectiontherapytotreat43casesofTMJDSandgotagoodresult.Thecontrolsincludetwogroups,thesimplerinsingandthetraditionalinjection.Statisticalanalysisshowedahighlysignificantdiferenceintheefectsbe-tweentestandcontrolgroups.Inordertostudythemechanismofthetherapy,thetumornecrosisfactor(TNF)insynovialfluidof8caseswereassessed.ItwasfoundthattheconcentrationofTNFwasreducedmarkedlyaftertreatment.WethinkthatthenewtherapyisefectableforTMJDSandcouldbeusedwidely.
