98例儿童髋关节结核病灶清除疗法

Clearance of Focal Lesion for Hip Joint Tuberculosis in Children(with a report of 98 Case)

我院自1965年5月～1990年3月,共收治儿童髋关节结核98例。我们认为一旦诊断成立,应尽早采取滑膜切除及病灶清扫术,1982年以后,术后由传统髓人字石膏外固定,改用皮牵引制动,若术后有脱位倾向者加用两枚克氏针交叉内固定,可防止术后脱位,有利于护理及减轻患者的经济负担。本组随访时间最长者21年,最短者1年,平均7.8年。本文强调早期诊断,早期手术,更多地挽救关节功能。

The incidence of hip joint tuberculosis in children is quite high.If it is treated in time satis- factory results can be achieved.From May 1963 to March 1990 98 cases of hip joint tuberculosis in children were treated.It is suggested that synovectomy and clearance of focal lesion should be performed as soon as possible once the diagnosis was confirmed.Since 1965,spica cast was re- placed by skin traction for postoperative external fixation.If the affected hip tends to dislocate two crossed Kirschner pins can be used for internal fixation to prevent postoperative dislocation and be helpful to postoperative care.In this group the duration of follow-up is between 1 and 21 years with an average of 7.8 years.Early diagnosis and early operation are necessary in order to obtain better joint function.
