股四头肌腘绳肌等速肌力测试的信度研究

Reliability of isokinetic muscle strength test for quadriceps and hamstrings

本研究目的是确定Ｃｙｂｅｘ３３０型等速功能评定系统测试股四头肌、绳肌等速肌力，即峰值转矩（ｐｅａｋｔｏｒｑｕｅ）和伸、屈膝峰值转矩角度（ｊｏｉｎｔａｎｇｌｅｏｆｐｅａｋｔｏｒｑｕｅ）的可信度，为今后进一步研究提供参考。健康医学生４０名，分为同一检查者（Ａ）和不同检查者（Ｂ）重复测试组，依次用每秒６０°、１８０°、３００°的角速度测试等速伸、屈右侧膝关节的肌力，关节活动范围限定在９０°，记录股四头肌、绳肌峰值转矩和伸、屈膝峰值转矩角度，重复测试间隔７天。结果：①两组重复测试各速度股四头肌、绳肌峰值转矩的相关系数在０．７８～０．８９之间；②在Ａ组中，除１８０°／ｓ绳肌的复测大于初测峰值转矩并差异有显著性外（Ｐ＜０．０５），股四头肌、绳肌复测峰值转矩并非都大于初测峰值转矩，且两次峰值转矩间的差异均无显著性（Ｐ＞０．０５）；③６０°／ｓ的屈、伸膝峰值转矩角度的相关系数分别是０．４９和０．７５（Ｐ＜０．０１），１８０°／ｓ伸膝是０．３９（Ｐ＜０．０５），其余差异均无显著性。本研究说明用此等速运动装置测试股四头肌、绳肌快、中、慢等速肌力和在慢速伸膝时峰值转矩角度的结果稳定、可靠，可作为临床和康复评定或研究的客观指标。

The purpose of this study was to determine the reliability of Cybex 330 isokinetic dynamometer for assessment of quadriceps and hamstrings isokinetic muscle strength,that is,the measurement of peak torque and joint angle of peak torque in knee. Forty healthy medical students were tested and divided into group A and group B. Each subject was tested two times. The interval between both tests was 7￣10 days. Group A was only determined by the same investigator and group B was tested by two investigators in turn. The ROM of knee was fixed at 90°,then the angular speed such as 60°, 180° and 300°/sec was tested and recorded respectively. Result indicated: 1. the correlation coefficient of peak torque value of quadriceps and hamstrings at each speed in both groups and double tests was 0. 78-- 0. 89 (P<0. 01 ). 2.group A,except the peak torque value of the second test at speed of 180'/see was higher than that of the first test in hamstrings,the others were not changed. 3. the correlation coefficient of joint angle of peak torque at angular speed of 60°/sec in knee flexion and extension was 0. 49 and 0. 75 respectively (P <0. 01), that of angular speed of 180°/sec in knee extension was 0. 39 (P<0. 05). The rest wasn't different.It is suggested that the Cybex330 isokinetic dynamometer is a reliable index for rehabilitation assessment.
