岭南挑筋法治疗强直性脊柱炎临床研究

A CLINICAL STUDY ON THE TREATMENT OF ANKYLOSING SPONDYLITIS BY NANLING MUSCULAR-FIBERS-PRYING-OUT THERAPY

为客观评估岭南挑筋法治疗强直性脊柱炎(AS)的临床疗效和安全性,采用多中心,分层随机对照方法进行临床研究。将符合AS纳入标准的198例患者随机分为治疗组(挑筋组)111例,对照组(药物组)87例,以综合临床指标及临床症状的分级量化分值作为评价指标进行临床观察。结果治疗组和对照组综合临床疗效有效率分别为43.23%和12.64%,症状疗效的有效率分别为81.98%和45.96%,两组比较均有非常显著性差异(P<0.001)。治疗组的早期患者症状疗效优于中期患者(P<0.05),治疗后腰痛、背痛、胁痛、骶痛、僵硬、坐蹲起立、行走、睡眠情况、颈痛、腰脊、髋和颈项活动症状较治疗前明显改善(P<0.001,或P<0.05)。2个月后随访,治疗组的复发率明显低于对照组(P<0.05)。治疗组治疗后除2例患者针挑口偶有瘙痒外,余无任何不良反应。表明岭南挑筋法能消除或减轻腰背僵痛等临床症状,改善脊柱功能活动,且安全无副作用,是治疗AS较理想的方法。认为其适用于强直性脊柱炎以腰背僵痛为主的早、中期,尤以早期效果明显。

The objective of the paper is to evaluate objectively the clinical therapeutic effect and safety of Lingnan muscular-fibers-prying-out therapy (LMFPT) on ankylosing spondylitis (AS). The clinical study was carried on with the multi-central, stratified, random and control methods. One hundred and ninety eight AS cases according with the admission standards of AS were randomly divided into the treated group (n=111 cases) and the control group (n=87 cases), which were treated by LMFPT and by the drugs, respectively; the comprehensive clinical indices and clinical symptomatic indices were observed. The results showed that the comprehenssive clinical effective rates of the treated and the control were 43.23 % and 12.64 %, respectively and the symptomatic effective ones were 81.98 % and 45.96 %, respectively with a significant difference between the two groups (P < 0.001); the early-staged cases in the treated group had an advantage over the middle-staged ones in the therapeutic effect (P < 0.05), the symptoms of the treated group, such as pains in the waist, back and hypochondrium, sacralgia, cervicodynia, stiffness, sitting, squatting, walking and sleeping function, and movements of the neck, back, and hip, were much improved after the treatment compared with the pre-therapeutic ones (P < 0.001 or [WTBX]P < 0.05); the follow-up two months later showed that the recurrent rate of the treated was significantly lower than that of the control (P < 0.05), there were no bad reactions in the treated group after the treatment except two cases felt a little itchy in the muscular-fibers-prying-out site, suggesting that LMFPT functions to remove or remit the pains and stiffness in the back and to improve the spine movements with no side-effects whose indications include early- or middle- staged AS, especially the early-staged, being a relatively rational therapy for AS.
