维拉帕米的血液动力学研究

为探讨维拉帕米正性肌力舒张作用的血液动力学机理，对２０例左室舒张功能障碍的患者，于静脉注射维拉帕米前、后进行了左、右心导管和超声心动图的同步研究，结果显示：（１）维拉帕米具有显著的负性肌力收缩作用；（２）维拉帕米可显著改善左室的松弛性、僵硬度和舒张早期充盈；

Toclarifythehemodynamicmechanismofthepositivelusitropicefectsofverapamil,leftandrightheartcatheterizationaswelastwo-dimensionalandDopplerechocardiographyweresi-multaneouslyperformedin20patientswithleftventriculardiastolicdysfunctionbeforeandafterin-travenousinjectionofverapamil.Theresultshowedthat:(1)verapamilexhibitedasignificantnega-tiveinotropiceffect;(2)verapamilimprovedleftventricularrelaxation,chamberstifnessandearly-diastolicfilingsignificantly;(3)leftventricularfilingimprovementafterverapamilinjectionwasmainlyduetothereductionoftheleftventricularafterload.Itisconcludedthatthemajorhemody-namicmechanismofverapamil-inducedpositivelusitropicefectsistheimprovementoftheleftven-tricularmyocardialdiastolicpropertiesandthereductionoftheleftventricularafterload.
