菲立磁增强MRI对局灶性肝病的检出价值 被引量:1

摘要 目的 :着重探讨菲立磁增强扫描对局灶性肝病的检出价值。方法 :收集经螺旋CT增强扫描及菲立磁MRI增强扫描且临床、手术病理证实局灶性肝病 2 6例 5 7个病灶 ,采用分组盲法ROC曲线分析对比评价菲立磁增强MRI与螺旋CT增强扫描对局灶性肝病的检出价值。结果 :联合平扫MRI+增强MRI与其它三种方法之间比较 ,Az最大为 0 975 0 ,P <0 0 5 ,单独菲立磁增强MRI的Az=0 9315虽然大于平扫MRI的Az=0 92 2 6 ,但两者间差异无显著性 ,P >0 0 5 ,CT与其它三种方法之间比较 ,病灶检出Az =0 82 70最小 ,P <0 0 5。结论 :菲立磁增强MRI对肝局灶性病变的检出价值优于螺旋CT增强扫描。联合分析平扫 +菲立磁增强MRI的肝局灶性病灶的检出价值优于单独分析菲立磁增强法。 Abstract: Objective: We compared radiologist's pefformance on combined unenhanced and feridexs- enhanced MR imaging with their performance on helical CT enhanced, unenhanced MRI, and feridexs - enhanced MR alone imaging for the detection of local hepatic lesions. Methods: MR images and CT seans obtained in 26 patients with 57 local hepatic lesions were analyzed with ROC analysis. The imaging of patient were divided into 4 groups including combined unehanced and feridexs - enhanced MR imaging group, helical CT enhanced group, unenhanced MRI group, and Feridexs - enhanced MR alone group. Results: The combined approach resulted in larger area under the ROC curve (Az = 0.9750) and accuracy (86.8%) ( P 〈 0.05), as compared with the others methods. Although the area under the ROC curve of Feridexs - enhanced MR alone group is larger than that of unenhanced group, there are no significant difference between this two groups in statistics ( P 〉 0.05). The area under the ROC curve of CT group is the most smallest as compare with the other groups (Az=0.8270, P〈0.05). Conclusion: Feridexs - enhanced MR imaging was more accurate than enhanced helical CT scan in detection of local hepatic lesion. The combined analysis of unenhanced and feridexs - enhanced images was more accurate in the detection of focal hepatic lesions than was review of feridexs - enhanced images alone.
