脐带扭转螺距值及脐动脉血流参数与新生儿阿氏评分的关系

Relationship Between Snail Lengh of Torsion Cord and Umbilical Arterial Blood Flow Parameter and Neonate A-family-score

目的探讨脐带扭转螺距值及脐动脉血流参数与新生儿阿氏评分的关系。方法选择在我院待产的孕妇发生脐带扭转46例,孕周在37～41周之间,测量胎儿脐动脉血流参数、计算脐带扭转1周节段的长度和脐带直径,即脐带扭转螺距值;胎儿出生后立即进行阿氏评分,观察其与新生儿窒息的关系。结果脐带扭转螺距值越小(<2.0)及脐动脉血流S/D比值增高(>3.0),胎儿宫内窘迫发生的机会越大,新生儿阿氏评分越低。结论脐带扭转螺距值及脐动脉血流参数可作为产前及产时评估胎儿危险度的一项重要指标。

Objective To explore the relationship between snail length of torsion cord and umbilical arterial blood flow parameter and neonate A - family - score. Methods Forty - six gravida suffered of torsion of cord in our hospital were evaluated. Pregnant period was ,37 -41 weeks, the umbilical arterial bleed flow parameter of fetus was measured ultrasonographically. The segmental length for a round of torsion of cord and diameter, which was the snail length of torsion of cord of the belly band were calculated. Neonatal A- family - score was proceeded immediately after delivery. The relationship with neonatal anoxia was closely observed. Results The shorter the snail length of torsion cord （ 〈 2.0） had the higher of the umbilical arterial blood stream S/D ratio （ 〉3.0）, the increased occurrence of the fetal distress in the womb and the lower of the neonate A - family - score. Conclusion The snail length of torsion of cord and the umbilical arterial blood flow parameter is an important predictive marker in evaluation of the fetal antepartal and intrapartal risk.
