摘要
目的研究甲状腺肿瘤细胞增殖指数PI和凋亡指数AI的关系;探讨甲状腺肿瘤的发病机制及PI和AI在鉴别甲状腺良、恶性肿瘤中的诊断价值。方法用免疫组化Envision法和TUNEL法分别检测76例甲状腺肿瘤细胞中Ki67和AI表达。结果①39例腺瘤和37例腺癌PI和AI表达在腺瘤和腺癌组间比较差异有统计学意义(P<0.05或P<0.01);②15例滤泡性腺瘤(FTA)和15例滤泡性腺癌(FIC)的PI表达组间比较差异有统计学意义(P<0.05);AI表达组间比较差异有统计学意义(P<0.05);③15例乳头状腺瘤(PTA)和15例乳头状腺癌(PTC)AI表达组间比较差异有统计学意义(P<0.05)。PI表达组间比较差异无统计学意义(P>0.05)。④9例嗜酸细胞腺瘤(OCTA)PI表达和7例嗜酸细胞腺癌(OCTC)PI组间比较差异无统计学意义(P>0.05)。9例OCTAAI表达和7例OCTCAI表达组间比较差异有统计学意义(P<0.05)。⑤AI表达强度和PI表达强度呈正相关(rs=0.595,P<0.01)。结论甲状腺癌细胞的增殖与凋亡相关,甲状腺癌细胞的AI表达增高不表示有良好生物学行为。AI表达率和PI表达率可以作为鉴别诊断甲状腺良、恶性肿瘤的参考指标。
Objective To study the relationship between PI and AI in thyroid neoplasms. To study diagnostic value of difference of PI and AI between benign and malignant neoplasm of thyroid. Methods expression of Ki67 antigen in 76 cases neoplasm of thyroid was examined by immunohistochemical method (Envision) and TUNEL. Results ① Among the 39 thyroid adenomas TA, and 37 thyroid carcinomas TC, The difference of PI and AI between 39 TA and 37 TC were significant respectively(P〈0. 05 or P 〈0. 01).② The difference of PI between 15 FTA and 15 FTC was significant(P〈0.05). The difference of AI between 15 FTA and 15 FTC was significant(P〈0.05).③ The difference of AI between 15 PTA and 15 PTC was significant(P〈0.01). The difference of PI between 15 PTA and 5 PTC was nosignificant(P〈0.05).④ PI of 9 OCTA were 1. 69 ±1.30, PI of 70CTC were 2.21 ± 2.57. The difference of P1 between 9 OCTA and 7 OCTC was nosignificant(P〉0.05). AI of 90CTA were 0. 03 ± 0. 10, AI of 7 OCTC were 0. 54±0. 63. The difference of AI between 9 OCTA and 7 OCTC was significant(P〈0. 05). ⑤ Among the 37 thyroid carcinomas and the 39 thyroid adenomas, correlations were observed about AI and PI(rs = 0. 595, P〈0. 01). Conclusion Among the 37 thyroid carcinomas, correlations were observed about AI and PI. The increases of AI do not means satisfactory biologic behavior. PI and AI are assayed for helping to identify benign and malignant neoplasm of thyroid.
