摘要
目的探讨三维彩色多普勒超声成像技术在诊断膀胱恶性肿瘤中的应用价值。方法应用二维和三维彩色多普勒超声分别对经手术后病理证实的24例膀胱恶性肿瘤患者进行检查,观察肿瘤的数目、大小、形态、位置、肿瘤内部血流分布情况以及有无穿支样血流。结果三维彩色多普勒超声成像清晰,对24例41个肿瘤结节全部作出诊断(100%);二维彩色多普勒超声发现35个肿瘤结节(85.37%),二者差异具有显著性意义(P<0.05)。三维彩色多普勒超声显示肿瘤结节大多形态不规则,呈“菜花样”。肿瘤结节内部均见血流信号,其中26个肿瘤结节可见穿支样血流,10个较大肿块内部见杂乱的血管缠绕形成血管团,5个肿瘤结节仅在其基底部见少量血流。二维彩色多普勒超声显示肿瘤结节内部血流信号呈星点样、短棒样。结论三维彩色多普勒超声在显示肿瘤的外表形态、肿瘤数目、内部血流分布情况以及穿支样血流方面明显优于二维彩色多普勒超声,可以提供更多的诊断信息,值得进一步探讨研究。
Objective To investigate the diagnostic value of the three-dimensional color Doppler ultrasonography in malignant bladder tumors. Methods Twenty-four patients with bladder tumor (41 tumor nodes) were examined by two-dimensional uhrasonography and threedimensional uhrasonography. The tumors' number,shape,location, intratumoral vessel and perforating vessel were observed. All cases were confirmed by operation and pathology diagnosis. Results Three-dimensional color Doppler ultrasonography images were clear and stereoscopic,all the 41 tumor nodes were found by it (100%). But only 35 tumor.nodes were found by two- dimensional color Doppler ultrasonography (85.37%). There was a significant difference between the two examinations( P 〈0.05). Irregular shapes were showed in most of the cases on three-dimensional color Doppler ultrasonography. The blood signal was found in all tumor nodes. Perforating vessels were found in 26 tumor nodes. The vessel masses were found in 10 tumor nodes. In the rest 5 nodes,there was a little blood signal at the base of them. In the same cases, two dimensional color Doppler ultrasonography showed that there were blood dots or blood sticks in the tumor nodes. Conclusions Three-dimensional color Doppler ultrasonography is superior to two-dimensional ultrasonography in showing the shape, number, intratumoral vessel and perforating vessel of tumor. More diagnostic information can be gained by three-dimensional ultrasonography. It is valuable to be used in clinical.
