E-钙粘蛋白表达与胃癌临床病理学关系的研究

Relationship Between Expression of E-cadherin in Gastric Carcinoma and its Clinicopathological Features

目的:探讨E-钙粘蛋白(E-cadherin,E-cad)的表达与胃癌的浸润、转移及预后的关系。方法:应用SP免疫组化方法检测76例胃癌E-cad的表达。结果:正常胃粘膜上皮中E-cad全部表达。Ⅰ期E cad的异常表达率为19.6%,而Ⅱ、Ⅲ、Ⅳ期的异常表达率分别是57.2%、62.1%和69.3%,Ⅰ期和Ⅱ、Ⅲ、Ⅳ期间有非常显著性差异(P<0.01)。伴有淋巴结转移者51例中异常表达为74.3%,而无淋巴结转移者40.2%,两者间有非常显著性差异(P<0 .05)。在进展期胃癌中按肿瘤生长方式分组,结节型E cad异常表达率41.2%,而溃疡浸润型异常表达率67.3%,两者间有显著性差异(P<0.05);高中分化胃癌E cad异常表达率32 .0%,而低分化胃癌E cad异常表达率69.6%,两者间有非常显著性差异(P<0 01)。癌细胞浸润深度达粘膜、粘膜下层和肌层者E cad异常表达率为30.1%、38.2%,而浸润至浆膜及浆膜外者异常率为67.3%,两者间有非常显著性差异(P<0.01)。胃癌组织中E-cad的表达与癌组织分化、浸润和转移密切相关。结论:E-cad与肿瘤的发生、浸润、转移有关,可作为一种新的肿瘤标记物,对判断预后有一定价值。

Objective ：To study the relationship between the expression of E-cadherin and invasion, metastasis and progression of human gastric cancer. Methods：76 cases of human gastric cancer were examined for E-cadherin by SP immunohistochemistry. Results：E-cadherin was present in all normal gastric mucomembranous epithelium. E-cadherin was associated with gastric cancer differentiation,invasion and metastasis （P 〈 0. 01 ）. Conclusion： E-cadherin is associated with gastric cancer differentiation, invasion and metastasis,which suggests E-cadherin as a new tumor marker.
