脑出血死亡时间与死因

摘要 为探讨脑出血不同死亡时间的死因,对341例脑出血死亡患者进行分析。结果表明,3天内死亡者 98.2%死于脑疝及中枢性呼吸、循环衰竭(168/171),仅个别病例死于消化道出血、心脏病变和其它原因:4-7 天死亡者中脑疝及中枢性呼吸、循环衰竭仍居首位(68/79,86.1%),肺内感染居第二位(4/79,5.1%);8-14 天死亡者中,脑疝及中枢性呼吸、循环衰竭所占比例降低(36/55,65.5%),肺内感染比例增加(9/55,16.4%); 15-21天死亡者中肺内感染上升至第一位(9/23,39.1%),消化道出血居第二位(5/23,21.7%),脑疝及中枢性呼吸、循环衰竭退居第三位(4/23,17.4%),多器官功能衰竭居第四位(3/23,13.0%);≥22天无死于脑疝及中枢性呼吸、循环衰竭者,消化道出血上升至死因的第一位(5/13,38.5%),肺内感染居第二位(4/13,30.8%), 多器官功能衰竭居第三位(3/13,23.1%)。结论:脑出血后在不同的时间死因有所不同,临床抢救治疗时应制定相应措施。
